    Customer Service Follow Up Form

    This form can be used anytime you're trying to help a customer who needs a follow up from customer service or a manager.  Once you've submitted this form, you can know that the customer service team will take care of things from there. Be as detailed as you can, so we are able to help the customer in a timely manor. 

    Tell the customer: "I'm going to take all your information and create a support ticket for you and our customer service team will be in touch as soon as they are able.  They are in the office Monday through Saturday from 8 to 4."

    Please include item, price per, quantity and estimated total
